We may have a prize waiting for you!

How the Prize Draw works:

First, you need to take part in the Follow-up study. Sign and send the consent forms back to us. As soon as we receive these forms we will send you a letter detailing how to complete the online questionnaires and stating the required completion date.

Those who fully complete the questionnaires by the completion date will gain one entry into a prize draw for an iPad2. You can gain one bonus entry if your parent also completes their questionnaires in time! Your entry number for the draw will be your DAWBA ID number (this number will be in the letter about the questionnaires which you are going to receive).

It's important to note that we will be holding several iPad2 Prize Draws. You will only be entered into one prize draw, depending on when you are supposed to complete your questionnaires by. For example, if you have to complete your questionnaires between September and November 2011, your entry will be in the running for prize draw number 2.

Please make sure to check the date and results of the correct prize draw for your entry. Prize draw results will be available on the same day as the draw. As well as the winning ID numbers being made available on the website, winners will be contacted directly either by telephone, email or post
